Senior Living Placement & Transitional Care Services
Moving into an assisted living community, memory care residence, or long-term care facility can be a significant adjustment for older adults and their families. At Housecall Specialists, we help make this transition easier by coordinating the medical evaluations, required documentation, and ongoing healthcare needs associated with senior living placement.
Our medical providers, Marina Ryan and Katie Hnis, under the medical direction of Dr. Daniel Boss, work closely with senior living communities throughout Northern Palm Beach and Martin Counties. Whether a patient is moving from a private residence, hospital, or rehabilitation facility, our team helps families and facility staff prepare for a safe, organized transition.
Medical Evaluations & New Resident Admission Requirements
Florida’s licensed senior living facilities have specific health assessment and documentation requirements for new residents. Housecall Specialists assists with the medical portion of this process, helping families complete necessary paperwork and avoid unnecessary delays in admission.
Our services include:
New Resident Medical Evaluations: Assessing a prospective resident’s medical history, current health conditions, medications, mobility, and overall care needs.
Florida Admission Documentation: Completing required medical forms, including the AHCA Form 1823 for assisted living residents, when applicable.
Medication Review & Reconciliation: Reviewing prescriptions and helping ensure accurate medication information is available to the receiving facility.
Chronic Disease Management: Evaluating ongoing conditions such as diabetes, hypertension, heart disease, and dementia to support continuity of care.
Care Coordination: Communicating with families, facility staff, hospitals, rehabilitation centers, and other healthcare professionals to help facilitate the transition.
Ongoing Primary Care: Providing medical house calls after admission to help residents manage chronic conditions and address new health concerns.
Supporting Assisted Living & Memory Care Communities
Our services extend beyond new resident admissions. Housecall Specialists works with assisted living facilities, independent living communities, memory care residences, and group homes to support residents’ ongoing medical needs.
By providing primary care directly within these communities, our providers help reduce the challenges of transporting older adults to outside appointments. We focus on early identification of health concerns, medication management, preventive care, and timely follow-up after hospitalization.
For residents with dementia or limited mobility, receiving care in familiar surroundings can also help minimize the disruption associated with leaving their community for routine medical appointments.
Additional Services for Senior Living Facilities
Housecall Specialists also supports participating senior living communities with select employee health and educational services, subject to availability.
Facility Employee COVID-19 Testing: Coordinating or providing testing services when available and appropriate.
Employee Influenza Vaccinations: Helping facilities arrange seasonal flu vaccinations for staff.
Employee In-Service Education: Offering educational programs on relevant healthcare topics, which may include infection prevention, chronic disease awareness, fall prevention, and recognizing changes in residents’ health.
These services are designed to support facility staff, promote resident well-being, and encourage communication between healthcare providers and senior living teams.
Continuity of Care Before and After Placement
The transition to senior living doesn’t end once admission paperwork is completed. Residents may need medication adjustments, specialist follow-up, additional therapy, or ongoing monitoring of existing medical conditions.
Housecall Specialists works collaboratively with families and facility staff to address these needs and support a smooth transition. Our goal is to help residents receive consistent, personalized healthcare while reducing avoidable emergency room visits and hospitalizations.
Housecall Specialists serves Jupiter, Tequesta, Juno Beach, Palm Beach Gardens, North Palm Beach, and surrounding communities throughout Northern Palm Beach and Martin Counties.
Whether you are a family member preparing for a loved one’s move or a senior living administrator seeking medical support for residents, our team is here to help.
